Referring a Patient

If you feel that one of your patients may benefit from the services provided by St. John's Hospice, then please refer using the West London Specialist Palliative Care Referral Form, which you can download here.

Referrals should be faxed to 020 7806 4041.

If you are unsure as to whether a referral would be appropriate, please contact our community liaison nurse who will be happy to discuss it with you. The community liaison nurse can be contacted on 020 7806 4065 within office hours.
