What is a Community Palliative Care Nurse? The Community Palliative Care Nurses will visit people at home to offer support and advice. We are all qualified nurses with a specialist background in palliative care. The emphasis is upon maintaining your comfort and the aim is to help you to cope with the effects that your illness may be having on your life.
How do we work?
- Each patient referred will be allocated a named nurse, who will assess their needs, plan and co-ordinate their care.
- Primarily, our work involves visiting you at home, we also have close links with the inpatient unit and day centre so can refer you to these services if needed.
- We offer support, advice and assistance to people who have life threatening diseases. For people with other diseases we may be involved for a period of assessment.
- We work closely with others involved with caring for you at home. For example District Nurses, GPs and Care Workers.
How often will the nurse visit? This depends on needs and will be discussed and planned with your key nurse.
Will I have to pay for this service? No, the services provided by St John's are offered free to all patients.
How can I be referred to a Community Palliative Care Nurse? Talk to your GP, District Nurse, Care Manager or Social Worker and they can refer you to the service. Once a referral is received, one of the nurses will contact you to arrange a visit.
