Hospice at Home extends the care provided by our Inpatient Unit into the homes of patients who want to remain in familiar and comforting surroundings during the later stages of their illness. Those benefiting from Hospice at Home are often elderly and highly vulnerable members of our community.
The service is managed by the Hospice at Home coordinator who is also the team leader of the community Macmillan nursing team. The care is provided by four Hospice at Home health care workers. Our carers also work in our Inpatients Unit which means that they have broad experience and knowledge of hospice care as well as being well trained in caring for people in their own homes.
The aim of the service is to offer a comprehensive specialist palliative care service at home by augmenting existing community support services. It is not intended to replace statutory services, but to complement existing care packages.
Our carers assist with all aspects of personal care from bathing and preparing meals to escorting patients to appointments for treatments such as chemotherapy or easing patients' discharge from hospital to their homes. Another important part of the service is to lessen the strain for family members who are caring for the patient.
