About this role

We're looking for a Clinical Nurse Specialist Trainee to join our team as we extend our service delivery to 7 days per week 8 am to 8 pm.

The aim of this post is to provide the opportunity for the post holder to develop knowledge and skills in all aspects of Community Specialist Palliative care nursing whilst working with the Hospice Community Specialist Palliative Care Team.

In order to progress to a Band 7 Community Clinical Nurse Specialist position the post holder will need to achieve competencies based on a framework of the following 4 distinct domains:

  • Communication
  • Leadership and Management
  • Quality
  • Clinical Practice


This will include the post holder being closely supported through a structured programme of supervision, reflection (written and verbal) and attendance at training courses such as advanced communication skills and clinical assessment skills.

What are the key duties?

  • Work under the direct supervision of one of the Community Clinical Nurse Specialists in Palliative Care in maintaining a caseload of patients.
  • Attend regular clinical team meetings, locality meetings and Multidisciplinary team meeting and be able to discuss the management of patients with the clinical team.
  • Assist the Community Clinical Nurse Specialist team in the continuous assessment and evaluation of the needs of patients and families, recognising physical, psychological and social needs, and agreeing ways in which these might be addressed.
  • Work with the wider clinical multidisciplinary team to provide ongoing assessment of current patient needs, effective symptom management and monitoring and evaluation of effects of prescribed treatment.
  • Demonstrate effective communication with colleagues at St John’s Hospice , acute and community healthcare professionals liaising with other palliative care/oncological teams/units and other specialist professional teams in order to facilitate continuity of care for patients with life limiting illnesses.
  • Ensure accurate written and computerised patient records are maintained including inputting of patients onto the electronic end of life care register Universal
    Care Plan.
  • Demonstrate skills in working with families facing loss and identify any particular risk factors that are of concern and communicate these to the Hospice Social Work and Bereavement Service.
  • Participate in clinical audit as part of the hospice audit programme.
  • Draw on research and literature to ensure all practice is evidence based.
  • Develop and practice advanced communication skills including working towards end of life care discussions and advance care planning with patients.
  • Maintain effective communication with patients, carers and professionals to ensure seamless service delivery.
  • Provide communication pathways to key stakeholders in the patient’s care across health and social settings.
  • Actively seek out educational activities.
  • Attend in house teaching sessions.
  • Develop clinical competence and expertise in assessing patients and carer ’s physical and psychological specialist palliative care needs in the community.

Am I the right person for this role?

  • You will be registered with the NMC.
  • Educated to degree level.
  • Have experience in palliative and end of life care in a community or inpatient setting.
  • Be experienced in working in a MDT.
  • Be passionate about developing into CNS in Specialist Palliative Care.
