Referrals to St John's
We welcome referrals for patients and/or family members living within our catchment area. This includes the London boroughs of Westminster, Brent, Camden, Harrow, Hammersmith & Fulham, Islington, Kensington & Chelsea.
We accept referrals for patients aged 18 and over living with life-limiting or terminal illnesses with specilaist palliative care needs.
Referrals must be made by a GP, hospital doctor, nurse specialist, community nurse, or any other health or social care professional already involved in a patient’s care. We cannot accept self-referrals, except for child bereavement support where young people aged 16 and over can refer themselves.

Please note: In working hours (Monday - Friday from 8:30 am – 4:30 pm) referrals received in the morning are triaged the same afternoon, and those received in the afternoon are triaged the following working day. If urgent, please follow up the referral by calling us on 020 7806 4040 and we will attempt to triage the referral as soon as we receive it. Out of hours, urgent referrals will be discussed with the consultant on call.

Please note: This service will be soft-launching in Hammersmith & Fulham at the end of November 2025, making it possible to refer end-of-life patients as a priority. The service will be officially rolled out in January 2026 for all.

Download the formsOur physiotherapy, occupational therapies and complementary therapies are offered to patients using other services and their loved ones. As such, patients must already be known to our hospice to access this service.

Download the formWe welcome referrals for people living within Westminster who have lost someone through palliative care. We also support those who have lost someone who was cared for in our Inpatient Unit even if you live outside of Westminster.

Download the formAny parent, carer, teacher or other professional working across health and social care services can make a referral and young people over 16 can refer themselves. The child or young person must live in or be educated in the borough of Westminster to access this support. Their loved one does not need to have been cared for by our hospice to be eligible but the bereavement must relate to palliative care.
With parental consent, professionals can refer children and young people in writing using our referral form which is available to download below. Please return the completed form by email to nhsnwl.stjohnsreferrals@nhs.net.
