About this role

We're looking for a Hospice@Home Clnical Support Nurse to join our team, supporting our exciting expansion into Hammersmith and Fulham. The Hospice@Home Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patient’s home by Healthcare Assistants.

The post holder will be responsible for the coordination of our Hospice@Home service which provides home care for patients living with in our catchment area of Westminster, Kensington and Chelsea, Hammersmith and Fulham and South Brent.

You will work with the Hospice@Home Nurse Manager to ensure that all team members participate in professional development and are supervised appropriately within their remit of competency.

What are the key duties?

  • To provide support and guidance to a team of healthcare assistants specifically employed for H@H.
  • In conjunction with the H@H Nurse Manager,  support training for Healthcare Assistants employed as part of the service.
  • As required assist in coordination and collection of relevant data for evaluation, commissioning and audit purposes.
  • As required assist in the promotion of the service to external agencies.
  • Work closely with the H@H Nurse Manager and Community Palliative Care Nurse Manager for the overall management of the service.

Am I the right person for this role?

  • NMC Level 1 or higher qualification.
  • Post registration in qualification in palliative care.
  • Proven experience in oncology/palliative care.
  • Experience of working with community healthcare professionals.
  • Proven experience of nursing at a senior level (desirable not essential).
  • Good organisational skills.
  • Excellent report writing and presentation skills.
  • Data collection skills.
