Hospice@Home

Our Hospice@Home service provides specialist palliative care at home so you can remain in familiar and comforting surroundings during the later stages of your illness.

Supporting you at home

Our Hospice@Home service was designed to give you the choice of where and how you wish to be cared for during the advanced stages of your illness and to die at home if that is your preference.

Our care is tailored to each of our patient's individual needs and is delivered by our team of experienced Healthcare Assistants. This can include providing practical care such as washing and toileting, supporting with nutritional needs as well as with moving and handling, recognising changes in your symptoms and contacting relevant professionals on your behalf. We can also provide emotional support and advice.

We can provide a minimum of four hours of care per day through to 24-hour care, depending on your needs. Our care is usually provided for up to two weeks. Once referred we will organise a time to assess your needs and will work with others involved in your care to ensure the best plan of care is put in place.

What we can offer you

  • End-of-life care at home, enabling you to remain in familiar and comforting surroundings during the later stages of your illness. This includes facilitating your discharge home from hospital or hospice and preventing unwanted admissions to these settings.
  • Emergency periods of respite for family or carers to help lessen the strain of caring for a loved one at home.
  • Occasional escorts for hospital appointments to support vulnerable patients.

Who we see

We recognise that illness does not discriminate - it affects people regardless of their age, gender, ethnicity, sexuality or beliefs. We care for any person living with a life-limiting illness - including cardiac, respiratory, or renal conditions, HIV, cancer, motor neurone disease, multiple sclerosis and Parkinson's.

All of the care we provide is completely free of charge and available to anyone aged 18 or over living within our service catchment areas.

How to access Hospice@Home

We welcome referrals for patients living within Westminster, Kensington and Chelsea, and the South part of Brent. Referrals can be made to us by your GP, hospital doctor, nurse specialist or community nurse or by other professionals working across health care services.

If you are a health care professional and wish to refer a patient to our services, please download and complete a Specialist Palliative Care Referral Form and Hospice@Home care plan and risk assessment. Please return the completed documents by email to nhsnwl.stjohnsreferrals@nhs.net.

On the weekends and bank holidays, there will always be a member of our team on call who will be available from 9 am – 5 pm and who can be contacted on 078 8436 2241. Outside of these hours please call our hospice reception desk on 020 7806 4040 where you will be able to speak to an on-call member of staff for advice.
