Bringing hospice care home: A first for Hammersmith & Fulham

We are delighted to announce that we are expanding our renowned Hospice@Home service into Hammersmith & Fulham, bringing 24-hour specialist palliative and end-of-life care directly into people’s homes, a first for the borough.

For more than four decades, we have delivered high-quality, holistic care to people living with life-limiting illnesses, supporting both patients and their families. Each year, we care for more than 4,000 people across North London through a wide range of services, including our Inpatient Unit, Wellbeing Centre, social work and welfare support, bereavement counselling, and community nursing.

Now, with support from NHS North West London Integrated Care Board (ICB), we are able to extend our Hospice@Home service into Hammersmith & Fulham. The ICB will fund 50% of the project’s costs, enabling us to introduce this vital service in an area where this level of home-based hospice care has never existed before. This investment recognises both the growing demand for palliative care and the increasing pressure on NHS services. It is estimated that around 3,000 people in the borough could benefit from this support1.

We are currently the only hospice in London offering a fully 24-hour, wraparound Hospice@Home service. This service gives patients the dignity and freedom to choose how and where they are cared for, allowing them to remain in the comfort and familiarity of their own home. In 2024 alone, we delivered nearly 20,000 hours of Hospice@Home care, providing both respite and end-of-life support for patients and their families.

The service will soft-launch at the end of November, initially supporting people at the end of life, ahead of a full rollout in the new year.

To ensure the service is sustainable and accessible to everyone who needs it, we are now calling on the support of the local community to help fund the remaining 50% of ongoing costs. Today, we are launching a fundraising appeal, alongside a new ‘Sponsor a Healthcare Assistant’ regular giving campaign. Funds raised will also be supported through our upcoming gala dinner at the Wallace Collection.
